Bridging the gap from crisis relief to long term stability

Operation Follow Through is a wraparound support initiative developed within USA Cares to bridge the gap between short term financial assistance and long term stability for veterans and their families. The program is scheduled to officially launch in mid January, starting with follow up support for veterans in Kentucky and thoughtfully expanding to additional states we serve as capacity allows.

While USA Cares is widely known for emergency financial support, Operation Follow Through makes sure veterans are not left on their own once immediate needs are met. Our team provides structured, proactive follow up after financial assistance so we can check in on stability, identify ongoing needs and offer support before a new crisis develops.

Through brief wellness check ins, mental health navigation and connection to community based services, we work alongside veterans to build a path toward lasting stability, not just short term relief.

If you have received financial assistance from USA Cares, your case manager may invite you to participate in Operation Follow Through as part of your ongoing support plan.

How Operation Follow Through works

  • 1 After you receive financial assistance from USA Cares, our team schedules follow up outreach to check in on your stability, stress level and goals.
  • 2 We use brief screening tools and wellness check ins to learn how you are doing with safety, housing, work and support systems.
  • 3 With your permission we provide warm handoffs to community based services, mental health care and other resources that fit your needs and preferences.
  • 4 We stay engaged over time to support long term stability, not just a single moment of crisis.

About Operation Follow Through

Operation Follow Through is designed to make sure veterans do not fall through the cracks once immediate financial needs are addressed. The program provides structured, proactive follow up with veterans after assistance, with a focus on mental health navigation, connection to community based services and sustained engagement over time.

During scheduled check ins our team uses brief screening tools, including anxiety and wellness questions, to better understand how each veteran is doing across key areas of life. These conversations are guided by trauma informed practice, respect for choice and a commitment to meeting veterans where they are.

When additional support is needed, we offer warm handoffs to mental health providers, counseling services and other community resources based on individual needs and preferences. Our goal is to reduce barriers to care so veterans can access help that feels safe, culturally responsive and aligned with their personal goals.

Operation Follow Through is also informed by evidence that pairing temporary financial assistance with ongoing wraparound services leads to stronger housing stability, better engagement in care and improved well being for veterans and their families.

We are intentionally building a network of mission aligned mental health providers who value collaboration, continuity of care and veteran centered practice. By creating reliable referral pathways and staying connected with both veterans and providers, we aim to support coordinated care that benefits everyone involved.
